**Over-the-counter medications may be given for 10 days with short term medication form (1 time each year per medication excluding antibiotics). A doctor's order is needed to administer cough drops however!!**
Short term medication form is available in the clinic to be filled out by a parent for a medication that is only needed for 10 days or the course of a short term dose of antibiotic. After 10 days a doctor's order is required to continue giving medication. Students may also only have short term order for a particular medication 1 time each year.
(Example: If motrin given for 10 days and no order received from doctor, parent cannot complete another 10 day for motrin until next year).
Long term medication form is to be filled out by the doctor and signed by the parent as well. It is for medications the child will need longer than 2 weeks. i.e. ADD/ADHD medications, lactose intolerance pills, etc.
All medication must be kept in clinic! If parents need to administer medications during day please come into clinic and child will be called to clinic--no medications may be given in classrooms or cafeteria!
All medications must be delivered and picked up by parents/adult--nurse is unable to send medications home with children per NEISD protocol and for safety reasons.
